All the ketchup recipes I've seen as well as all the ketchup in supermarkets has either sugar or some other sweetener like high fructose corn syrup added to it. Why? It's supposed to be salty/savory and getting rid of the sugar/corn syrup would only make it healthier. Unless I'm overlooking something, there's literally no reason to add sweeteners.
Average Americans can't stomach anything that isn't full of HFCS, there's your answer.

tomatoes can be pretty acidic, the sugar helps balance the acidity

At most you'll typically see 1/3 to 1/2 of one side of a freezer section devoted to pizza.
