The twist to MGSV is that there is no other big boss, kojima's big ruse was was tricking the seasoned mgs veteran into having the saje mental illness that big boss had. The paz sequence proves that venom snake is capable of much more complex hallucinations than the red herring color distortion you expedience at times. Ishmael makes no sense, there is no reason for big boss to protect his body double. The truth cutscenes are cheesy as fuck and don't fit at all chronologically with the crash and boss leaving the island, and for big boss to have been in the same hospital for nine years we'd either have to believe that boss and big medic were in comas for the same amount of time, or that big boss elect to sit in a hospital contracting bed sores while big medic took a nap. The real story of MGSV is of a mentally ill big boss so shellshocked by losing motherbase that he creates an alternate scapegoat identity of big medic to continue contributing to the war atrocities that he was directly responsible for perpetrating (everyone knew about motherbase, its child soldiers, how many men big boss sent to die, worst of all everyonrecopying him). The "from the man who sold the world" tape is schizophrenic ramblings, tldr there is no body double and kojima tricked all his fans into being mentally retarded.

To be honest I'm not even convinced ocelot is real, its confirmed that big boss has halucinated his presence (paz again), and it easily could have been big boss torturing huey, talking to himself on tapes (he halucinates paz's voice on an empty tape, ocelot completely ignores his mentions of a man who helped him)
While less evidenced, ocelot could easily be projection as well considering how big complex big boss's hallucinations can get. Dont forget how eerily different ocelot is in this entry.
