Over time, as mankind's command of its realm expanded, our technologies and our own actions took the places that were once held by mythological beings. Nowadays, parents indulge their children with gifts, displacing the old Santa Claus.
However, there exist parents who snap and do unthinkable things to their children. Every year, our news media broadcasts stories of parents who went over the edge. Almost like they were possessed by a demon...
With no Santa Claus to reward the righteous, and no punishment for the wicked, both forces are unleashed indiscriminately against good and evil alike. For every naughty child rewarded, a counterbalance must exist.

It would be pretty easy to kill an asthmatic. Give her an empty inhaler he saved. Open up a bottle of nail polish remover or a can of paint. It wouldn't be hard.
Ramona's autopsy showed her lungs were hyper inflated, which is caused by over use of inhalers and the like. She had an asthma attack, and panicked/it wasn't working so she took a ton. She would have had to breathe in while it was spraying and hold it in, so there isn't any way art could have made her do that unless he was torturing her and forcing her to OD which is unlikely. I don't believe the recent stalker bullshit story, but this was clearly an accident. I've got asthma and I can't think of a way to force someone to overuse that stuff.

Considering evidence in the film I find it highly likely that Courtney was somewhat involved in the process and probably orchestrated the whole thing by hiring some low-end hitman and bribing the police.

Watch Nick Broomfield's interview of "El Duce" (AKA Eldon Hoke), taken from his documentary "Kurt and Courtney." He's the man who claimed, on the record, that Courtney offered to pay him to kill Kurt but he turned it down. He's not the most reliable witness but get this: Two days after he went on the record for Broomfield he was found dead on a railway track, hit by a train in the middle of the night.
