As far as lore goes, is there only one CyberDemon/Spider Mastermind?
Like, are there just two guys in Hell fighting over which of them is better?

Thy Flesh Consumed didn't ship with the original game. It's commonly seen as just an addon with no story. Just a few extra maps to kill stuff in.
I honestly wouldn't even say that the two times you fight the Spider are different Spiders. If Doom 4 lore is cannon, than they could be the same Spider but different universes.

They made that fight way easier in the HD version you idiot, of course you'd think that you were hot shit.
I wouldn't say I struggled but I think most died to her at least once because there is no way to see the mega death coming the first time. The fight constantly rewards you for getting rid of the zombie status by giving you a free regen bonus and then all of the sudden BAM if at least one of your character doesn't have zombie you auto lose.

For what? It's an okay game. A good 90% of this game is big, open world areas with nothing but fetch quests. Even the proper questline for each of the optional areas is dull and underwhelming.
It's a shame to because the story missions in this game aren't terrible. When you get sent to the doomed future, the assault on haven, the ball at the winter palace they're all really good missions. They just spent most of dev time making the big mmo areas and as a result there are barely any story missions.

Considering that Fallout New Vegas came out not long after FO3? And that NV is heralded as one of the greatest modern WRPG's, worthy of 1 and 2?
No. They certainly changed it for the worse in many aspects but the things FO3 did well were good (improved graphics, gun/sword play), and everything FO3 did horribly NV did absolutely right to make one of the rarest gems of its time.
FO4 falls into the same rut as FO3. It's status as an RPG is terrible, but as an openworld action shooter it's honestly really fun, and to its credit expands on FO3 in nearly every aspect. No one in their right mind will tell you FO4 is worse than FO3. Though FO4 also squanders the treasure traits/skills/SPECIAL system this is something that could be easily rectified in another NV-esque sequel, leading to, again, a timeless RPG if the FO4 engine were to be put in hands of Obsidian's dedicated team (and, given more than a year to make it).
