I think the maps are not where you the player goes but the extent to where people travel or observe.

The point is that everyone knows that even if they went with Fallout 1/2/Tactics style maps, it would still take like 20 games to cover all of America.
Even more now that they are doing one major city style maps. And even with that, it becomes a thing were you have to pick your battles.
There are a lot of large famous cities in America, but we will never visit all of them because it would take too many games to make, so the devs use the games they do make to talk about those places in lore, so we can know whats going on there, without ever actually having to go there.
For instance,
-We hear in New Vegas that Denver has been entirely abandoned by humans, and overrun with feral and wild dogs in such numbers not even the Legion could conquer the city. We also hear from Lanius that the area was almost devoid of human life outside the Hangdog tribe, which made the Legion's task of capturing the city difficult since they had no people to enslave to refill their ranks.
-We hear that Salt Lake City was entirely leveled in the great war by 13 nukes, that also caused the great salt lake to evaporate. The only civilization in Utah is in New Canaan, in the ruins of Ogden, the rest of the state is nothing but hostile tribes and raider warlords.
-Idaho and Wyoming get mentioned in a few ending slides, the Great Khans and White Legs can retreat to those states after the events of NV, suggesting there is little there in the way of any sort of powerful groups. The northwest in general is described as "wilderness" and "in ruins" and apparently lacks anything that could stand up the Khans, as they are able to forge a mighty empire out of the region. We also know the Followers of the Apocalypse are in the area, suggesting the presence of tribal groups like those that were in The Legion's lands, as the Followers are known to send missionaries out to those tribes to teach them, and learn their languages.

We hear in Fallout 3 that Philly is "in ruins" and "that dump", we also hear that the Liberty Bell is still supposedly intact, laying in the ruins of the city. Abraham Washington was going to hire a team to go retrieve the bell for his museum.
We also learn in The Pitt that Pittsburgh has become the most powerful/largest city in the post-war state of Pennsylvania, not Philly.
When Ashur gives his big speech to the people of the Pitt, he compares his city to the major wasteland of the region, including The Capital Wasteland, The Commonwealth, and Ronto.
He makes no mention of Philly, or NYC for that matters, despite the fact thay are closer to The Pitt then The Commonwealth is, seemingly because they aren't important or notable in the post-war world.
We also get little snippets of Atlantic City, Erie, and wherever the Abbe of the Road is, to round out some locations in other states in the East Coast that we dont get, as well back more backwater places like Point Lookout and Far Harbor to show what life is life in those kind of regions.
In short, they use little snippits of places to show what life is like in the larger immediate areas since they know they will never be able to show it all.
The map covers the areas those games covered in lore, not what we got to directly play.
