Well, the Andals are from North-Western Essos, where fair skin, light eyes, blonde, red and brown hair are prominent. They're essentially the Germanics of the series
The First Men are darker, though not as swarthy as the Rhoynish. They originally crossed from the Arm of Dorne though, so perhaps they're related to the Rhoynish closer than they are to the Andals, being both from mid-south western Essos. They're basically the Celts of the series, who have dark eyes, dark hair, and look a bit Iberian.
I think all these peoples originally come from Asshai though, which is the Ancient India of the series. This would parallel the migration of Indo-European peoples.
The Summer Islanders, Yi Tish and Ibbenese are their own separate ethnic groups. I think the Dothraki and the Ghiscari are related genetically. I don't know where the Valyrians got their genealogy from

I think part of the alias in the book is also for the kind of response you see from the small council in the episode. Qyburn's necromancy isn't exactly something they should be flaunting when they have so many enemies looking for a reason to come at them. Qyburn got his chains taken because of it and I'm sure if any Maesters caught wind of what he was doing there'd be some people coming after him. Particularly in a city where you've got the faith militant about causing such a ruckus. It's just kind of dumb/lazy writing on D&D's part and the further flattening of the plot in service of TV.

I thought the whole "Umbers refuse to swear loyalty" was kind of a sign of the throwing away of tradition we also see in the books post RW. Where the only thing that made oaths and swears have any sort of meaning was the fact that all of them would uphold them. That bread and salt wasn't magical, it was the idea behind it that made it so sacred. When that trust was destroyed for Robb Stark, it was destroyed for all of Westeros and I believe when he says "fuck kneelin and fuck oaths" or whatever it's a reflection of how the general population feels about such things after the RW.

When Boltons were besieging Winterfell, Theon was being driven crazy by some slug-horn type thing being constantly blown. That's easily explained when keeping the logistics of production in mind.
It's cheaper to have an actor do more scenes in 1 episode than the same amount of scenes in 2 episodes. And you can say a lot about D&D, but they get a LOT out of their budget. GoT is famous in TV-land for having excellent production values.
