So I just watched Kahn Academys video on Maxwells Demon and its quite obvious how he proves it in regards of how the demon will expend create more heat than will flow from the cooler to the hotter body. The second law of thermodynamics requires a system to be in thermodynamic equilibrium. Strictly, probably nothing in our world is ever in proper equilibrium. The example you mentioned (bringing two objects together and looking at t=x nanoseconds) is definitely not an equilibrium situation. I don't think they reject the existence of a disease, but they reject the way in which psychiatric disorders are categorized.
There's a lack of study on the mechanics of the human brain. As soon as we can learn more about the human brain a more found science can be established.

If I wanted to induce hypoglycemia (low blood sugar) quickly, painlessly, and with no sense of hunger, how would I do it?

They are. You sense of balance would say you were upside down because gravity would be pulling on your inner ear and your flesh the wrong way.
As for hearing, if you were weightless and blind, the phase difference in your ears would always tell you the direction regardless, but if you had an alarm clock on a shelf and a cat meowing on the floor, the cat would seem to be on the shelf, and the alarm on the floor.
