You can log in with your pixiv account. I think that's all it takes, but it's been years since I did that.
'Add to cart' is self explanatory. It will automatically take you to the cart; click the yellow button on the right (with an arrow pointing right) to go to checkout.
Credit card radio button is obvious. Select it, scroll to the bottom, same yellow button.
Field names are:
Card number
Name on card
Expiration date
CVV code
phone number
They should accept foreign cards without issue.
