HOLY TERROR was always supposed to be a BATMAN story. Not only that, but it fits on Miller's Batman timeline.

Hulk comics never sold that well, it doesn't really matter if he's alive or dead, no one really cares, people have taken to Cho fairly well too
They live through decades of adventures and they kill them in the most lame and unemotional ways.
Secret Wars has a lot of that too.
Instead of thinking they need better writers and artists for a comic to sell, they just cancel series and kill off characters.

The driving force behind the changing of Harley Quinn is that an editor at DC has a massive skankification fetish (see also: Mary Marvel, Supergirl, Duela Dent). Probably Eddie Berganza because he seems like a disgusting person in every other field of his life.
It also makes it easier for Hot Topic sluts to cosplay her.
