-in reality there aren't a lot of people in the world who have an overwhelming desire to kill another person, let alone torture them to death.
-the people in the world that would want to inflict that damage on another person typically doesn't have the intelligence or education to have the money to pay for something that would in theory have to be extremely expensive. it would have to be extremely expensive because if it wasn't the risk for the people providing the service wouldn't be worth it. and also they'd have to be paying the government and police to keep quiet.
