Time and patience.
That, or they draw the original design, pixelate it, then clean it up/stylize it. I would be very impressed to find a 3D animation program which keeps track of all of the "sides" of a subject, the respective colors it would be if turned around, and then in real time pixelate it just right.

is just a blind guess, I don't know anything about graphics, but I think that may be the product of porting "old techniques" developed for displays at very low resolution (and so big pixel size) and <=8 bit color depth to modern screen.
