the gloomy style of this pape makes me pine for a horror game, perhaps in a fallout world where big bad boys are scary and not just rare. imagine an evil brotherhood of steel, where ghouls are slaves and dissenting opinions are slaughtered nazi style. would be hard to pull off the horror aspect though without cutscenes...
Not out yet, but I'm a huge Zelda fan.

you could probably mod something like that in fairly easily. maybe make all or most factions hostile, increase ghoul spawns, permanent night, remove some of the goofier stuff like super mutie dialogue, radroaches, etc.
Alternatively you could just play STALKER. Those games get preddy spoopy
