Here's mine, it's a modified version of SP8 (I'm not sure if that's the name of the theme, I'm not on my pc right now) and unfortunately I ran into some problems trying to change the trackbar but I'll try to fix it one of these days.

It's a little of both, I used Columns UI and UI Hacker. I put the NG Playlist within a Panel Stack Splitter to make the scroll bar disappear
