Hardest one I can think of is Wizards & Warriors II: Ironsword. It's pretty much impossible to beat it without either cheating or knowing the game like the back of your hand.

I own that game. Got a real love hate relationship with it, there's no fucking way I can beat it legitimately and I've beated the shit out of Battletoads and Ninja Gaiden I and II. My friends 55 year old mom plays that game religiously and she can beat the hell out of it. I'm literally assmad.
