It's hard to explain without seeing it firsthand, but it was more or less made to be the most offensive and violent adventure game of all time and also serves as political commentary against people who think video games cause violence. It was ahead of its time in many ways - it basically accomplished what Spec Ops: The Line did about 15 years ahead of time. It's also needlessly over the top and disgusting in its content and about as subtle with its message as a brick to the face.
