Is Kanto rural and not very technologically advanced because it happens before most of the other games, or is it because the region is in the ass end of the world?

In pretty much every form of pokemon media that exists, kanto is always pretty shitty.
Technological convenience is pretty low. Compare any version of Kanto to Unova for example.
Even in the anime, it looks very shitty. Cities are small and pollution clogged. Their league facilities are really subpar compared to every other league. It honestly seems like some ex-yugoslavia shithole.

It's 'rural' because it was created for the Game Boy, and the technological limitations of the time could potentially be the reason for Kanto's general lack of technology. I mean, it has large cities, computers, and other such things. However it's very 'product of the time'.
Though if we're also going to speak about technological advancement, keep in mind that Kanto does have warp tiles, medical devices that can instantly heal your Pokemon with no issue, and even fossil restoration technology.

Kanto is pretty much fucked design wise due to how it was initially portrayed. All you can really do is use common sense to realize that the problem generally just stems from the game's perception. There's not much you can really do with that
This is all horribly similar to a belief a friend of mine has that there are 'no phones in Kanto' because the games don't show any in the overworld.

Pretty much all of its landmarks are within towns. The center of the region is literally a city surrounded by other cities on each side
Most routes are just straight paths to other towns.
Its probably the most urban region after Unova.

The thing is that the same look still carries onto the anime no matter how recent the portrayal of Kanto is. It's had a very consistent look.
