We know Kanto is connected to Johto.
We know Johto is connected to Sinnoh via Sinjoh Ruins and that Sinnoh lies north of Kanto and Johto.
We know Unova is connected to Sinnoh via cave.
We also know Kalos is connected to Sinnoh by train.
My theory is that ALL the regions are connected by a giant mountain in the center that makes up the northern mountains of Kanto and Johto, Mount Coronet in Sinnoh, the West Mountains of Unova, and the East Mountains of Kalos.
Here's my map work in progress, using ONLY sourced in game facts as evidence. Also included are the spin off regions.

The desert section up to Agate City is Arizona because that was in Colo. XD added the western part of Orre and the coast which more closely resembles Southern California and the gulf area, with some liberties with Citadark.
Sinnoh also has the Sakhalin area of Russia (though disputed with Japan) so it's not like they don't skirt borders for the sake of making a region.

On Pokedéx entries for Gen I and its remakes they mention things from the real world like China, South America, and even the Guyanas, but Raichu's and Gastly's Dex entries standout by their mention of Indian Elephant, which it doesn't have a Pokemon based on it. Donphan is an Elephant, but not an Indian Elephant.
