Even if that was real, that final evo is way too abstract and minimalistic to be the finalized design. They'd bedazzle the shit out of it with spikes/colors/etc before giving the OK on it.
They look nice but it's kinda like Ralts' evolution line. First Pokemon small with no eyes/eyes closed/eyes not visible,second evolution opened eyes and doing some kind of pose,third evolution gets blades (Gallade). But... I can live with that

I don't remember the whole story behind them (they were posted in a Serebii thread) but basically people on the design board would draw these digital slips and pass them through to Sugimori who would redesign them using their basic concepts.

This is part of the reason that I originally took a break over gen 3. Groudon and Kyogre on the covers looked *way* too complicated to be pokemon compared to everything else in existence at that point. They really *did* (and still do really) look like something from another series like Digimon and it just bothered me how busy they looked. Since then pokemon designs have slowly evolved into being a bit more complex but still nothing quite as bad as them or Emboar. I really wish that they'd find a way to use that original design somewhere, even if it's modified into a different type or something. It looks awesome, like some kind of walking volcano, instead of some robot-looking thing like Groudon.

I don't know, but I assume this happens with every generation. Some designs Sugi does himself, some are passed through a line of people before they reach him. Even some finished ones aren't even designed by Sugi, he just does the arts that are published. Everyone keeps talking about how Sugi said recently that he wants the series to go back to simple designs, so maybe he's had a change of heart. Styles change, and that's evident in his change of art.
