I've always kind of wondered if the Earth and Rainbow badge were switched at some point. One of them looks like a fucking leaf and the other one feels like it should be the final badge with the Japanese naming scheme. Maybe they decided the rainbow badge was too girly for giovanni.

Well it is implied that there are more than 8 gyms in most regions and 8 badges is just the minimum you need to get into the league.
He probably didn't earn that Cascade Badge. Misty never mentioned battling him, which means he must've got it from her sisters, and they were so fucking lazy they were literally giving the badges away just to avoid battles.
