For the mitsumeru mod with the no dim integration, do I need to get the base mitsumeru mod as well? Reading the pastebin it tells me that variants require the base mod but I don't know if the No Dim integrated version is a variant.
Is there any certain way for the pc character to get Unforgivable status towards an NPC, I've had it in the past but it seems a bit hit or miss. even times when you catch your g/f and she chooses them it hasn't shown up.

I don't get how characters are saved. Does the program just take whatever image and generates everything? I've seen some pictures that seem to be screenshots of an anime yet the editor can open up the file with the proper looks. How is everything stored in a png?

It's stored as metadata. If you were to re-save the image in an image editor it WOULD get corrupted and would no longer be usable by AA2.
What is appendage I and II? My uncensor mod came with it. Is it the reason why the uncensoring has color mismatch and looks weird?

I'm pretty sure that it's through the file's metadata. In short, the PNG format (and a bunch of others too) is capable of having basic text data formatted in different "labels", much like MP3 and similar audio files. Like:
Frontal Hair: 65
Side Hair: 24
Personality: 4
And so on. The cards simply save these variables and then the game loads them and assigns them to the correct asset (the hairs, the personality's scripts, etc.).
This isn't really new. It's been used since at least 3D Custom Girl, and it's actually a really convenient way of storing and sharing custom characters.
