I'd play Io if his stats weren't so garbage. He's too easy to nuke down, and in pubs you can't expect your team to put in a ton of effort to keep you alive.
Up until you get to Very High skill Io is borderline unplayable, and even in VHS he's only viable in specific lineups. He's too niche and too self destructive for your average game of dota.

The mana nerf to his Q was pretty huge too. Made it so it costs too much to spam it in creep waves and the changes to neutral camps made it so he can hardly farm as well.

I don't think ALL should be implemented, but some certainly should. Purification is his one spell that's super good at higher ranks but comparatively useless at rank 1 or 2, so maybe make it at least mana efficient so you can choose for raw healing/damage or mana expenditure.

I don't play a ton of Dusa, but whats the generally opinion on Bloodstone here for her? The soul booster is really fucking good since armor is so shit on dusa, and the mana regen seems really appreciable since it's effectively more Hp regen, that gets even better as get deeper in the game. also, ever since memeback, pudge just isn't the same hero. it used to be that seeing a 6-0 pudge early in the game meant the game was over, and that's what drove a lot of picks.

If you are playing against smart people they will start trying to focus you down in teamfights. I usually go soul ring/tranquils (his mana costs are pretty low so it should do but arcanes are obviously needed if no one else can get them and to prepare mek and greaves later) and then cheap-ish utility items. Glimmer/forcestaff/euls are all good. Get the bare minimum items to survive and let the other greedier support get his stuff, you don't really need much, only levels. If you meant starting as in the very first, just wards/courier/tango/clarity or two and a smoke.
As for the other question, I usually try and gank safelane early on (after say, two mid ganks to get the enemy supports on-edge). Try and get the mid/offlaner to smoke with you and the other support when the first towers start going down and get important pickoffs. Place unconventional wards in the jungle to help you with this, not the usual hilltop-shit. Oh and people are usually careless with the courier. Depending on the mid, you can usually get a courier pickoff between 2-3 min just by smoking behind the enemy t1.
