If you have thumb buttons in the mouse you can either add 2 extra keys on your action bar, or you can do what I did and bind one to ctrl and one to alt and add a lot of space comfortably on your keys.
I use 1-6, Q, E Z and X for my Classic Mouseover healing macros.
Shift + those key binds for combat
Ctrl + those binds for defensive cool downs
And alt makes the spell cast on me.
I could probably be more efficient but I'm lazy.
