Yeah, most of it is just stylistic choices where they've made armor that looks too small for a limb to be inside it. I think McCree's the only one that for sure has a prosthetic, his character sheet calls it "mechanical arm" and it's designed in a way that definitely wouldn't accommodate an arm inside.
