Skull Face was probably a concept back then. If it helps you sleep at night, there's heavy implication that Skull Face's origin story is just a ruse and the face he has in is a mask. Some of the soldiers alluded to him as a costume Halloween party with a backstory of a comic book villain. That's a big contrast to the other guards that said there's no way that's a fake.
Skull Face's face in the concept art looks a bit like Ocelot to be honest. The mustache though could be a bit like Campbell as his concept art in MGSPO has a mustache and easy eyes

It works better than anything else I tried. Since 1.00+ staff are useless on their own (slow, can hardly take damage) the Raiden suit is the only thing that makes it do able. You can just run past the first deck before the invincibility wears off, dive down the ladder by the connecting bridge, run through the tunnel and around the side of the next strut, etc.
Any DMG or STUN weapon doesn't do a lot against their armor. Since it shoots gas, doesn't matter where you aim with the pistol thingy.

Skullface is a master bluffer. Listen to the Secret Recording of Skullface and Code Talker [2] for some idea just how much his plans hinge on bluffing. In fact, this tape is chronologically the first time Skullface mentions an English strain of the vocal cord parasites. In this same tape, he reveals that the parasites may be transmitted through water, describing the destruction of Mfinda Oil Field and how it led to the village downstream becoming infected. In this tape also, Code Talker comes to the realization that Skullface has been infected with the parasites. Skullface reveals that “all the world’s languages are already lost” to him. Even so, he speaks gratuitous amounts of English. In fact, if he ‘’had’’ developed an English strain, it would probably take from him the last language he could communicate in. When he leaves Code Talker’s cell, it is revealed that there was never any soldier waiting to hear the bell, which demands the old man’s complete attention. Code Talker doesn’t even have the presence of mind at that point to consider that maybe there was no English strain, either.

There was no English radio transmission in the Devil’s House, yet during the jeep ride Skullface shows you a canister containing two vials, and an empty slot, and tells you it’s the English strain. Snake later says, “there were three.” He never thinks to question if there were actually any parasites inside the vials (Code Talker’s purple vial showed that they’re visible to the naked eye), because his mind is consumed by the empty slot, wondering what happened to the “third vial, ” no matter whether it even existed or not. In Skullface’s death scene, you can get a good look at the remaining vials, and see that they contain only green fluid, no little parasite floating around at all. Venom glances at each of the two remaining vials before tossing them, but even then, his mind is on just how “close to” him the third might be.

Skullface told her he infected her with the English strain before she went rogue, and she believed him, conveniently preventing her from revealing any details of his activities or organization. The vocal cord parasite may be transmitted through saliva emitted during speech, coughing, sneezing, et cetera. Speech also triggers the mating pair to copulate, and the eggs to hatch into larvae that migrate into and eat the host's lungs. So, if Quiet believed she was infected, she would remain silent, right up until the end, when she speaks English to direct Pequod to the landing zone. What happens to Venom, who is within salivary range when she does this? Not a thing.

Zero told Ocelot that he would understand the "new snake" situation when he got to Cyprus. This means that the surgery to make the other man into a duplicate was already complete before Ocelot started his guard duty. Zero was infected by Skullface in 1977 (Report to Big Boss: Zero has been Attacked [1] references 1977 NYC blackout). 2 years after GZ. He visited Snake in the hospital Ocelot was guarding after this. The "other man" had already been given the face of Big Boss at this time (Zero: “Which one is he?” Doctor: “We... did just as you instructed.”). So, Ocelot hypnotically implants the persona of Big Boss into this “medic,” implants a suggestion of the self-image of the medic into Big Boss, and hypnotizes himself to think that the medic is the real Big Boss. Step A of the plan is complete. Now the world waits for them to awaken.

So, what is the "surgery" that the player character undergoes in the start of the game, when we decide what we want the avatar to look like? It's real surgery. It's the plastic surgery that Big Boss undergoes to make him look like the medic so that his enemies won’t find him. The doctor was telling the truth. Big Boss gets plastic surgery long after his double got surgery to look like him. Throughout the game, BB actually has the appearance of the avatar you make in the beginning of the game, and he's hypnotized to forget his identity. But, his subconscious still remembers what he used to look like, so that's how he appears to the player, except in very specific situations (reflection in ACC, the mirror in "truth," which I'll cover later, and gameplay afterwards). He's told he's Big Boss, but there’s a lot of not-so-subtle hints he picks up on that lead him to question that statement. In essence, the context he's surrounded by makes it impossible for him to recognize the truth, even when confronted with it every time he looks in a mirror, or is saluted to by his men. It’s Doublethink. Those he comes into contact with still call him "Snake" or "Big Boss" while subtly implying they suspect (or know) it isn't actually the real thing (Miller, Skullface, Emmerich, AI Pod). In fact, in the hospital, one of the soldiers gets a real good look at Venom's face (after Ishmael's IV bag leaks), and doesn't immediately radio in or show any other reaction indicating he found who they were all looking for.

During the truth, the doctor holds up the mirror and we see the face of the medic. This was what Venom expected to see after having that personality implanted into him. Then he undergoes plastic surgery, and after this he is told to “forget his past” and his mind compensates by “seeing” the face of Big Boss, the only other face his fractured mind remembers having. After all, he couldn’t have the same face after surgery, right? Yet, in some situations, when his mind isn’t paying attention, you can catch glimpses of the real face he wears, in his in-game shadow, which is missing the ponytail, or in the reflection of the ACC window. And of course, after “Truth” you can see it any time you want. And then, of course, Quiet comes in and kills the nurse and doctor, and gets a call on the radio: “Not yet, the patient in the next bed saw my face. … Of course, consider it done.” To her, Venom ‘’is’’ the patient in the next bed as she doesn’t see the face of Big Boss but the one he’s actually wearing, that of the medic. The order given over the radio was most likely was to eliminate the witness, so she immediately made to do so before Ishmael, who’s in a bed surrounded by Star of Bethlehem flowers as if to say “I’m Big Boss, please don’t assassinate me,” jumped her.

So what’s the deal with the horn, didn’t we clearly see that the medic jumped in front of Big Boss in the helicopter and caught it? Not in Ground Zeroes we didn’t, it’s left somewhat ambiguous. Only in a “flashback” in TPP, which could easily have been a false memory.
It would be typically Zero, to hide Big Boss in plain sight as "Big Boss.” Anyone dangerous enough to pose a real threat to the man would know what he looks like and how he operates, and wouldn't waste their time with a pretender. Even Skullface doesn't expend much time or resources trying to eliminate Venom, until he becomes too much a thorn in the side. The first mission you encounter him and his mech, and the skull unit is actually used as a hit squad rather than a guard unit, is the honeybee mission. At this point, Skullface is probably just following up on a report he heard about a mercenary operating under the alias "Big Boss." And when he finds Venom? He's obviously not impressed. He takes Venom for a small-timer and a phony (even sarcastically referring to him as "Boss," quotes included in the subtitles). Then he nonchalantly leaves his mist unit behind to mop up witnesses.

So, what happened to the other man? The one that still looks like Big Boss? The one that guards Venom as he escapes from the hospital in Cyprus? He was hypnotized to believe he IS Big Boss. Under hypnosis, he is a more willing collaborator for Zero than the real Big Boss ever would have been. Eventually he becomes the leader of Foxhound, while still quietly putting together Zanzibarland as per the idea Ocelot planted in his mind, without Zero or his AIs knowing about it. When The Patriot AIs decide that they no longer have use for the real Big Boss, they begin Op. Intrude N312 and, later, N313 to dispose of him and his “Outer Heaven.” Ocelot catches wind of this, breaking his self-hypnosis, remembers which Snake is which, and breaks the hypnotic suggestion he gave the doppelganger. The tape that Venom receives came from the phantom. It's a warning, and it reminds Big Boss of his real identity, allowing his conscious mind to recognize his changed face, which he then touches and pokes at, as if to confirm it's really there, like it's the first time he's seen it. After "Truth," you can replay any mission with the avatar, and he will take the place of Venom Snake even in cut scenes, because he was always there, and now Big Boss can remember it that way. Diamond Dog soldiers, despite each having unique facial features, do not get this treatment.

The Intrude N313 tape explains a plan that will allow the real Big Boss to escape. The Phantom takes Big Boss's place in Outer Heaven, while the real Big Boss gets out, has his face restored to match his restored persona, and takes over building the military nation of Zanzibarland. In Metal Gear 1, Solid isn't surprised by Big Boss's cyborg hand and shrapnel horn because the "Big Boss" he encountered didn't have one, it was the same "Big Boss" who commanded him in Foxhound.
In Metal Gear 2, Kessler relates to SS a rumor that BB was rebuilt with cybernetic limbs after the fall of Outer Heaven; A bit of shrapnel lodged in his skull wouldn't be unbelievable either, considering the man was thought dead in the bombing of OH. At the end, he gets burned alive by Solid Snake. Venom has a robotic arm and, in the release Trailer for TPP, Snake walks down a hallway, and as he does his appearance changes, from Naked Snake, to the Big Boss of Peace Walker, to Snake of Ground Zeroes, to Demon Venom, to a skinless horned face surrounded by flames (foreshadowing his defeat in MG2), to skull face. I believe that last transition is metaphorical, but that each of the others represents phases in the life of one individual.

Why wasn’t Miller suspicious of Venom the first time he saw him, if he was wearing someone else’s face? He’s fucking blind, that’s why. He recognizes Snake only by his voice, which is damned ironic for English speaking players considering the change in actor, though it works fine for the Japanese audience. The parasites he’s been infected with may allow him to “sense” others around him, as Code Talker does, to a limited degree, but he can’t make out fine details like facial features, as he hasn’t had time to learn to master them.

But, what of Eli’s DNA test? Eli is a clone, but he was also genetically modified. His DNA can’t be used to gain access to the Patriot AIs in MGS4, so it’s clearly not going to match the real Big Boss. Read up on chimerism (which can result in false negative paternity test results) and you’ll find that it’s more common in people born from in-vitro fertilization (as the clones were) and especially when multiple zygotes merge into one (recall Liquid’s ridiculous statement about there being eight Les Enfants Terribles, but that six were “sacrificed” to make the remaining two stronger; what he might have actually been describing was merging the embryos until only two remained). Ocelot and Miller also describe that Eli was “acting strange” when the tests were taken, just like when he was on the helicopter over OKB Zero. Perhaps it was actually Tretij Rebenok’s blood they sampled, having taken on Eli’s appearance. If Eli didn’t want to be identified as a clone of Big Boss, because then he’d be taken back to Cipher, maybe he cheated on the test. Ocelot shrugs off Eli’s obsession with Venom because he wants Venom to “believe” he’s the real BB. Ocelot knows Eli is one of the clones (reference “Les Enfants Terribles” tape), and he “knows” Venom isn’t the real Big Boss, so the results don’t surprise him. Miller questions Eli’s behavior because he knows about the clones; however, the negative DNA test makes him suspect that Venom might not really be Big Boss.
In the post-ending conversation between Ocelot and Miller, Ocelot “reveals” to him that Venom is a copycat. Miller then pledges to aid “the phantom,” Venom, in future confrontations between Snakes, which suits Zero and Ocelot’s plan to hide BB in plain sight just fine.

So, there’s just one loose thread. The Man on Fire, Volgin. Even though the “Big Boss” fake wakes up prior to Venom (per Truth tapes), Volgin doesn’t awaken and cause the plane crash until Venom regains consciousness and we hear the radio transmission about the plane (In the Doublethink tape, Ocelot tells “Big Boss” that Venom was not “actively conscious,” but this is merely to avoid raising his suspicion regarding identities). So, why does the Man on Fire freeze during the side op to recover his body, if Venom really is Big Boss? Consider, that the only other time he gave a shit about Venom specifically besides the hospital when you’re in close proximity to either the decoy, or Ocelot, whose treachery also played a part in his demise, is when he’s under Skullface’s indirect control. And consider also that at the time you find his remains, Venom is having doubts about his identity as Big Boss (as you, the player, must be by this point). If Volgin can sense the “spirit” or mind of Big Boss, as his reaction when Venom woke up indicates, then this confusion, combined with Venom’s altered appearance, means that Volgin does not recognize even a spark of the Naked Snake who did him in, and his rage cools to disappointment, cooling his body along with it. This is ultimate proof of the effectiveness of Zero’s ruse, that even a man who can see through mere appearances, couldn’t recognize Big Boss.
