Also you might want to drop a few points from STR into DEX given your weapon choice and drop LCK to 30 since Hollow infusions each give you 5 levels in LCK. Drop the Dark Sword as well, the Carthus is much better for your build and you don't need two stright swords since you got Anri's.
Maybe you could try getting the Warden Twinblades instead.
Also change your class to Warrior, I believe you might end up with a few levels to spare into DEX.

And really Dancer is just as bad, once she loses her super grab move she's much easier to handle.
I really started to hate how From had to give EVERY SINGLE BOSS a buff at 50% health. It was good for Gundyr to show players to expect shit like that and Abyss Watchers sets up a pretty rad 1 on 1 but after that they just got sad and predictable.
Look at DS1, you had bosses that could buff themselves and gain damage+moveset but the only boss with a real second phase gained iconic status for it. From clearly forgot that bosses are meant to be a feature of the level, not spectacles that define the entire game.
