People with Arrhythmia typically don't live past 30. His lifespan is probably the shortest in Lilly's route because he suffers multiple heart attacks but hasn't developed his body at all. Shizune's, Hanako's and Rin's are all on the same level in regards to his health so he would have similar lifespans. Emi's route has him combat his condition through exercise so it's the best outcome for him.
That said, Japan has higher life expectancies than other developed nations and there might be better treatments and medications in the mean time.

Well that is certainly one of the themes of the novel, it's not the main one.The kind of person you make Hisao determines how the story unfolds: An aggressively outgoing and committed person will end up with Shizune, A person determined to spite their disability ends up with Emi, A neutral but impassioned person will be together with Rin, And someone defined by their trauma but faces the world is meant to be with Lilly whereas the one who hides will find an equal in Hanako. Someone who is none of these things will be as unhappy as Kenji, but that's not to say that the other routes don't have their downsides too. There is a lesson to be learned in each.
