Pluto's poison is the primary cause of his current state, and he should not look as old as he does in his 40s. In Doi's mind, Kiyoharu was once like Jonathan. Tokyo is separated from everywhere else by the dome, which Mikado rests on top of. The rest of the world does exist since America and other places get mentioned in Kasumigaseki and other places by NPCs. Time moves slower in Tokyo than in Mikado, but which place is like everywhere else, if one of them even is, is not said. I'd guess that time was slowed down in Tokyo by the dome or proximity to the reactor though.
As for why no one from Mikado has explored beyond the dome: ┐(ツ)┌

I haven't played it myself yet, but from what I've seen, a lot.
There are many new events and some locations are slightly modified. For example, in Inogashira Park there is an event where you can save a woman from Zombie Cop, and there is now a toilet in the small red building, where you can find Hanako-san as an optional boss.
You can choose your partner from the original Heroine, Yuriko, and new Law Heroine character. Also, when Thorman presses the button, you'll get teleported to different locations, depending on which heroine you pick.
More demons and NPCs. Nuff said.

Nocturne / Lucifer's Call
This game is amazing, holy shit. I was sceptical at first due to the lack of futuristic/sci-fi elements present in other SMT games, but it's actually got a really good feel to it. The atmosphere seems pretty unique.

I'm replaying SMTIV for the first time since NA release and man, Jonathan and Walter's reasons to support their alignments seem even more forced than I remember. Almost every time Jonathan talks about preserving traditions and so on his arguments sound artificial and Walter goes from someone with decent ideas to full stupid in span of two-three scenes.
