Oh yeah, that is a definite problem. You will have to wait a long time until you can go to Kaer Morhen, a decent part of the main quest will be done at this point. (Basic wolven gear is probably already useless at that point, it was for me, anyway). Might go with your second best Witcher gear, if you want sth right now.

It leads to a (to me) unnatural quest progression, but of course it is doable.Do at least Family Matters in Velen and then do all of Yen's quests in Skellige. CDPR made an excellent choice when they decided to go with Triss as a main romance option in the trilogy. I am especially grateful for allowing me never to cheat on her, I feared they'll force me to romance Yen, but thankfully that never happened.
