Except for Kaer Morhen, which honestly seemed out of character, Roche's bro status is a bit overplayed by people I think. Most of his actions in TW2, almost all of them in fact, are entirely self motivated or for Temeria, not for Geralt.

Don't worry, you're not going to be left wanting when it comes to the size of the game. A regular playthrough can be up to 200 hours.
Skellige is a completely different region probably about 2/3 the size of Velen and you don't even go there until about halfway through the game. The Novigrad area is maybe 1/2 the size of Velen. Kaer Morhen is maybe 1/4 the size of Velen but is absolutely beautiful.
blood and wine contains a part where geralt helps a witcher from the griffin school who had his medallion taken and turns out to be almost a thousand years old.
Geralt also has the choice of rescuing a female alchemist from his past.
Griffin witcher explodes as soon as he gets his medallion and destroys half the city.

I let Dijkstra kill Roche and Thaler. That was so out of character though, they keep him close to the books till that moment. Felt pretty good about the political ending with that choice, pretty much the best one imo.

So I'm reading all the books as prep for playing the games, and I'm awfully confused about the origin worlds of humans and elves.
First I thought that humans were an invasive species that appeared without warning at the conjunction of the spheres, which happened a few hundred years ago. But then, elves start talking about how they had been watching mankind evolve for millions of years, so I thought that people and elves migrated over together during the conjunction. But then we see that the elves apparently had some sort of interdimensional timeshare system set up, and a unicorn shows Ciri a mountain of human skulls from when the King of the Alders first set up shop.
So what the fuck is going on? Did Auroch purge the humans "native" world, with all the humans in "our" world being refugees that migrated during the conjuntion? Where is the elves native world? Is it the one the humans invaded?

It's the best political ending (though there's not really any "good" endings on that front) but yeah like you said it's out of character. There are some other places where Geralt goes out of character too but they're pretty rare.CDPR normally doesn't make it so prominent.

Before the conjunction we have very little knowledge of the civilization of the elves and humans. The Aen Seidhe are fucks as they where the big race on the continent until the humans came across the sea some 500 years prior to the events in the books and btfo out of them. They saw their lands and civilization crumble so on top of feeling superior to everyone else they become very bitter. The Aen Elle are just dicks cause they like to conquer shit.

The Aen Siedhe were pretty accepting of humans when they first came and taught them more about magic, too. So they probably feel a bit betrayed. The superiority complex is relatively common with marginalized people as a defense of their ego.
