If I take a bunch of a "perk points" into the perk that increasing the effectiveness of stimpacks and radaways, that in no way implies I'm a doctor.

The reason this isn't done isn't that perks are somehow not able to function this way, it's that Bethesda is allergic to skill checks. Most skills were literally fucking turned into multi-stage perks, which they actually always were, except now you can only get them to a certain point depending on your level to prevent maxing guns immediately and massacring everything in one shot with a 10mm, and instead of getting points to spend on skills AND points to spend on perks you have to decide whether to put your level into the equivalent of 20-25 points in a skill OR a perk.
Skills are still there, they're just in a much better system. That's why the other guy was confused about what you were saying and thought you meant that having a high skill value reflected your character doing that thing a lot rather than reflecting that your character was able to do that thing well- Because your actual point is even dumber than the point he thought you were making. Medic 4/4 is the same as Medicine 100. Medic 2/4 is the same as Medicine 50.

There's no VATS, but targeted shots are really useful, especially late game. Aiming for the eyes is usually best on organics. Aim for the ganglion on floaters and the CPU or regulator on robots. But you shouldn't worry too much about this until you have higher skills.
Finish the game quickly or you get bad endings for every settlement. I just learned this the hard way.

If you window out a lot, itll cause memory issues.
There is also a semi-well known memory leak if you have FO4 on for 1-4 hours depending on your rig it gets slow and will crash if time somehow passes, IE no pip-boy or ESC menu to pause it.
Like seriously, windowing out to post, opening up pics and moving around screen shots is pretty harsh right now.
