I did an institute run first and there's literally no mention of killing everyone on the surface. They assume it's going to occur and they're taking measures to protect their underground society, but they're not going to actively murder everyone topside.

The Institute ending is pretty clearly a happy ending. The main character gets what he wants and human civilization has the opportunity to recover.
