1. Isaac must die to a Mulliboom in one of the Basement floors. Mulliboom seed: 6028 LLMK (Go into the large room above the starting room. You can easily die from full health in here to them.) Or if cum: JTMD EZPF (one is directly below - Normal difficulty)
2. Magdalene must die to her own bomb in one of the Caves floors. (Pyro Seed: ALFA 0NTZ)
3. Judas must die to Mom's Foot as Judas in one of the Depths or Necropolis floors. Good Seed: 8LDE 8XDX (Wait on using the Joker till the 3rd floor and avoid the devil rooms and you'll get the sacred heart. You also get the Sad Onion on the first floor.
4. Azazel must die to Satan in Sheol (his blood/laser form or hooves, NOT leeches or The Fallen). You must NOT use a seed.

Yeah they were, but in Zelda games your power increases at a predictable rate and eventually you have good enough weapons and enough hearts to make them a minor nuisance. In Isaac you are at the mercy of dice rolls for your power increases and they show up in early levels where you aren't likely to have much life or be very strong.

That is pretty much all there is to it.
1. Connects 2-4 rooms
2. Not next to boss rooms
3. All of the connecting rooms have clear access to the wall where the door would be

Regular secret rooms will always border at least two other rooms. Those rooms can be the shop, curse rooms, item rooms, etc.
Super secret rooms will always attempt to spawn as close to the boss room as possible. They will never border two or more rooms, and cannot border a shop, item room, etc.

For some reason the secret room is like 90% of the time next to the shop, and the super secret room is the majority of the time connected to the room just before the boss room. Fuckmund will probably change it up arbitrarily in the patch just to make sure we're chewing slowly enough.

Secret room rules:
Never next to a boss room
Almost always connected to 2-4 rooms
The order in preference goes 3 rooms, 4 rooms, 2 rooms, 1 room.
No connecting room can have rocks, spikes, fire, or pits completely blocking the way to the entrance.
The super secret room rules are:
Always connected to only 1 room
Never connected to a special room
Most often found near the boss room
Same final rule as the regular secret room.

The latter is definitely a thing. Basically, the Secret Room will priortize spawning next to 3, then 4, then finally 2 rooms that have unobstructed paths to the wall (no chasms or rocks).
The Super Secret Room will only spawn next to a single room, and tries to be as close to the boss room as possible.
The frequency of the normal Secret Room being near the Shop is just coincidance, possibly because of the Shops spawning, not the Rooms. Likewise it happens to be bordering a Curse or Challenge room rather often.
