Some characters retain information over saves and will "remember" you killing certain characters/doing things over and over
The last boss of the first end is an ebin creepypasta meme and forcibly closes your screen, saves/loads states and """overwrites""" your save file so you can't load before (but it also checkpoints you throughout the fight, making an already easy boss even easier and more pointless). The final boss of genocide run and pacifist run break the 4th wall a bunch to talk about how YOU THE PLAYER are a good person/bad person and how they are real and not code or whatever

4th Wall breaks are only good when they are subtle. If they start using it to deconstruct the game it begins walking a very fine line of having it be enjoyable or just come off as really pretentious.
Metal Gear Solid 2 is the only game I can think which did a major 4th wall break.
