Read MoF's prologue. Before the events of the game, Sanae went to the Hakurei shrine and told Reimu to shut it down (or surrender the shrine to the Moriya or something like that). Had Sanae not done that, Reimu wouldn't have gone to Kanako to tell her to cut that shit out.

That's the events of SA. Also they fuck up Satori's sister, that's the extra stage of SA, this has actual implications and is part of the reason for the events of HM and ULiL.
I don't think Kanako was involved in the UFO thing or Hidden Masquerade.
But these were just setup for HM where the shot hits the fan and people beat the shit out of each other over their religion.
Sometime inbetween Kanako comes up with zany schemes to make faith and they usually don't work out very well.

The point being that the Bad Ends are never canon, but that doesn't contradict the possibility that they could have happened. I mean if you're retconning stuff from the first game in the entire series, then nothing is free from scrutiny.
I think it's likely that they have that level of raw power, otherwise the ability to shut down Moon-destroying monsters outside of the Spell Card Rules (which presumably would need to be done pre-EoSD) would be nigh-impossible.

I'm saying I don't see why the orbs having radically different abilities, trasnforming into other creatures, when they literally never do so again, is probably not legit.
And thus I don't see that world destroying thing as something that's terribly legit.
Furthermore it is known that the Hakurei god is quite weak. It barely even exists at all.
It is also known that most of Reimu's strength doesn't come from gods. Although she's a shrine maiden, her powers are almost entirely spiritual, literally her most effective abilities being entirely her own (like floating away from reality).
So no I don't know what happened to the orb in HRtP, I don't even know if it happened at all, but if it did it's very unlikely had anything to do with the unknown god that survives on scraps of faith.

Frankly I always interpreted it as a side effect of Reimu going to these other worlds in HRtP. After all, it's outright stated and demonstrated that in that game Reimu can't actually use the ying yang orbs. She beats it around like a ball.
I don't remember whatever it was that was said about the shrine in PC-98 but Windows makes it sound like the shrine has always sucked. That, and Reimu's regular complaints and the fact that a god CAN'T be helped if the shrine maiden can't help them, and it's confirmed Reimu doesn't train at all so yeah, probably always weak. Possibly weakEST in HRtP given the aforementioned "can't use the orb".

None of this really contradicts Windows, and the bad ending of HRtP could simply be a result of the orb storing too much power.
And it's been shown in Windows that the orbs can do things outside of channeling her god's powers (like when Yukari modified the orb to act as a two way communicator.), so it's not exactly out of the question that it could do other things.

The only time I used extra lifes was when I first started playing 2hu all the way back from EoSD.

5) Eirin's too smart to get involved personally (she's a fugitive). Instead she sends Reisen with the Ultramarine Orb to get the heroines to go instead.
6) LoLK happens.
7) Doremy, bored with how the plan has been going, redirects the heroines to the real Lunar Capital (effectively betraying the 'Gu).

Sagume probably just figured she wouldn't use it. Sumireko planned on it, in a sense, but ultimately didn't, so Sagume's plan worked.

Story-wise it would go that Flandre one day decides she's going to go out to play and nobody is going to stop her. She runs into Meiling first who tries to convince not to leave but isn't able to stop her, so she decides to go along with to at least try mitigate the damage a bit. Then they go on a romp through Gensokyo where you're basically playing as the incident occurring, until Flandre finally tires out and goes back home.
Gameplay-wise it'd be similar to IN where Flandre/Meiling is the playable team with Flan as the unfocused fire and Meiling as the focused. Bosses would similarly fight in pairs using combined Spellcards (with somewhat combined themesongs). The first stage would be Remilia and Sakuya trying to stop them from leaving, though beyond that I haven't thought it out too much. Would run into similar pairs though like Yuyuko and Youmu, Reisen and Youmu, etc, eventually culminating in the final stage at the Hakurei Shrine against Reimu and Marisa.
The EX Stage would be playing as Meiling, running into Flandre fighting alongside Remilia, and being joined by Sakuya as the unfocused shot.
I even had sortof a name for this big autistic mess - "Colorful Flying Catastrophe".

That's how urban legends are born with the aid of the occult balls, however if I remember right the beings of Gensokyo fall in from the outside after being forgotten, aside from creatures like tsukumogami which are born through other means. They are sustained by "belief" (fear), though.
I'm not sure that the humans of Gensokyo can "create" beings through belief alone, if anything considering how rumors don't actually change anything or affect reality that seems to be totally not the case (example, the unknown kishin are those that fight hermits, unlike the belief that it's shinigami -- a widely held belief). There's also a belief in oni but they exist with or without "belief". They were nearly forgotten since they simply moved somewhere else.
I guess ultimately I'm saying I'm not sure how Gensokyo actually works in a lot of cases, like if the outside is responsible or the inside is.

Each puppet has two types, yeah. How you choose to fill it in really just depends on your personal preference, some just match the puppets they like to whatever slots are available whereas some will tend to actually look at what form they use and go from there.

From what I recall, the Moriya Shrine used to be in the outside world. Which is to say, our world. Unfortunately, modern society doesn't really worship gods or believe in magic enough to sustain the existence of supernatural beings. Because of this, Kanako and Suwako shifted the Moriya Shrine to Gensokyo instead so they could gain more faith and revitalise themselves. Unfortunately they were a bit too enthusiastic and went overboard with it, leading to Reimu telling them to stop stirring shit.
Despite leaving the outside world, Kanako still recognizes the importance of modern society's technology. Seeds of Unknown Form. It's one of Nue's abilities. It hides/changes what something looks like. The thing is, it doesn't turn one thing into specifically another thing. What happens is you look at it and based on your preconceptions, knowledge and experiences you will see it as what you expect it to be.
Nue tries to hide the things used to unlock Byakuren's seal using them, but it ends up making them look like UFOs to the heroes because they came here on a wild goose chase for UFOs. And since they wanted UFOs, they ended up collecting them and inadvertently helping unseal Byakuren anyway.
