I think CDPR really missed an opportunity by making Witcher 3 the last in the series. A spin-off game set in the Witcher universe where you play as a customised witcher recruit long before or past the main trilogy would have been so fucking cool.
You could start off as an inexperienced witcher, you wouldn't even have to be one of the Wolf School witchers since several Witcher schools were introduced in the books and games.
You could gain more knowledge about monsters the more you kill, travelling to distant regions in the Witcher world. Maybe we could finally see Zerrikania and Nilfgaard in a game.
The worst part is that CDPR is definitely capable of this but are busy with Cyberpunk 2077 which will seemingly never be released.
They never said they'd never make another one, it will just be a long time from now and will not feature Geralt as the protag.
Even if they don't make one, I'd much rather have them move on to fresh, interesting IPs rather than milk the franchise until everyone gets bored of them.
What we need is an MMO set in the witcher universe, so we can all be witchers and kill monsters together. Look how well it worked for elder scrolls and ESO.

Nigga, most people either hate ESO or don't give a fuck about it, it was forgotten almost instantly after release. To be honest I personally really enjoy playing it an think it's one of the best MMO's on the market, but it was definitely not a success.
And I don't think that "create your very own character" formula would work good for Witcher, one of the strongest parts of the series are well written, developed and interesting characters, not something straight out of the character creator and deprived of all personality. It could be made good by offering a shit load of choices, dialogue options and character personality development paths, but I would still much prefer to just see a game with a new interesting character. If you want to make your own witcher, player a pen and paper game.

We know from CDPR themselves that TW3 is the last Geralt game, but not the last Witcher game.
Thus we will see the world of The Witcher again, and not play as Geralt. It is very likely we will play as another witcher, and seeing how the biggest criticism of the game is the lack of character creation, it will probably be a custom one.

Geralt is interesting as a character precisely because he's different from almost every other Witcher. Most witchers are simple pragmatists who focus solely on doing the job, amassing coin, and never staying anywhere too long. Geralt's different in that he has many friends from across all races and has built a reputation for having a more human approach to the profession.

To be honest it is true that in books going for Yen makes so much more sense - But CDPR failed to introduce Yen in any reasonable way, not to mention you honestly need to be a beta faggot to chase after a woman for 20 years considering fact Geralt met Yen before he even knew about Ciri - And at the end of the game Ciri is a grown woman. (Geralt met with Yen in 1250, Ciri was born in 1251). And the final reasoning - Books have little to do with TW3 which should be treated either as a fan fiction or a franchise killer if Sapkowski decides to never touch the main story again.

Playing as a predefined character was part of what made The Witcher so great. A custom protagonist would be like shoving open world into Mirror's Edge because it's the cool thing people think they want.
