A wide variety of methods that got patched over the years.
I remember equipping a couple paladin auras in the quickbar and then just using the mousewheel to scroll through them at a rapid rate. This would cause you to lag out.
If you dropped any items on the ground while doing this they would stay in the game, on the ground, but your character would be rolled back and still have the items.
Blizzard eventually found a way to detect stuff like that and delete the dupes at regular intervals. However if you combined duped runes in the horadric cube they would become a legit rune and would not be deleted. That's basically why runes weren't worth shit while items like HoZ were very valuable.
At least that's what I remember. It's been a decade so the details might be a little off.
