They aren't any harder than what you're used to, it's a different kind of challenge though. Most patterns are made up of extremely dense slow moving bullets, most of the dodging happens in an extremely small area of the screen. Just search "touhou doujin download" or something like that, it's not difficult to find.

Normal difficulty, I tried C-1 for a while but recently have gone back to A-2, I'm just better at milking the first stage of that route.

Honestly I recommend watching the STG Weekly episode on it. It's where I got an idea of how to play the game, and what route to take and what spots to milk for resources.
It also covers imo the easiest route which is Luna > Milk > Star.

I'm not done watching it yet but I did notice he's trying not to hit the bosses with ice. I always go out of my way to try to hit bosses with ice since it gives you more motivation, is that a bad strategy for someone not playing for score?

What ice are you referring to, the one you freeze bullets with or the shot? Either way he's avoiding hitting the boss to keep them alive longer and freeze up more bullets.
As for survival I'd say doing that would be worth it early on or on patterns you know are easy/you wont die to. That way you can freeze up more bullets and build up your perfect freeze and motivation.
