So I've wanted to play Lisa for awhile but I've had some shit spoiled for me.
I know that at the end of the game Brad doesn't find Buddy, and I know that the second game you play as Buddy and kill Brad.
Should I still play this game knowing this or is it too late?

To be fair, you gotta consider the circumstances for each character. Brad is a horrifying monster in the eyes of many, many characters. His ability to fight manages to outpace Joy Mutants. People who didn't get the fuck out of the way when they saw him had a serious reason to. Especially towards the end of the game, where they found the only living female on the planet, and having their chance at her meant they could breed the next generation of human, or their legacy could carry on into an unknown future. Whereas buddy IS the last living female here, and all she has known through her life is isolation, threats of rape and murder, and a small number of trustworthy individuals she tried to make a family out of, but simply wanted to explore the world she didn't know.
I get this feeling that she was honestly only curious to see the state of the world around her, and that her genocidal rampage is a reaction to the confirmation that Brad was right on every single level. The world was a shithole, and she was a target by everything and everybody in it, and she realized that she needed to fight or be consumed by it. I don't think it'd be fair to keep her so morally ambiguous, because she'd just wind up getting hurtagainearly into the game and would wind up on her cynical rampage anyways.

Was Rando's "army" really that small or is it just that small solely for gameplay purposes so the fight wouldn't be too tedious, and also because implementing hundreds of sprites on screen would be almost impossible? I mean 30-40 men doesn't seem like a lot of people in retrospect.

I don't think a tremendous army could sustain itself. I think it's an "army" in respect that they are a vaguely organized unit of armed individuals with a similar goal, but they're probably around the size of a gang. Not to mention they were all over the place.

It was probably to scale for that specific area. Rando's army is all over East Olathe, fighting Buzzo's army, Joy mutants, keeping supply routes open, and doing general patrol. That island was secluded already, and on top of that, they had Rando himself there, who is more powerful than any of his subordinates. That would be more than a sizeable force for anybody that wasn't Brad.

Every enemy would be a potential ally as well. A core system would be choosing which 'allies' to sacrifice and when.
Joy would still give a 'nothingness' but instead of making the user a physical badass it would make their mind and personality entirely changeable. On Joy you could be anyone you want, think anything you want and it would be entirely sincere. Most of all the insane rules of FemOlathe would start to make sense.
FemJoy-Mutants would keep a relatively human shape. Their skin would remain intact while the bones and organs would turn to a sort of warped mush that would writhe about like a bag of maggots. The things would be perfect at mimicking voices, in fact they would seem incredibly trustworthy. However they would trick people around them into doing destructive, insane things.
FJoy-Mutants would also be surrounded by a clique. Fighting them involves finding out which one is the mutant before your party turns on you. Picking out the true mutant would be difficult task considering they imprint their psychotic 'personalities' onto anyone they hang out with for long enough.
