In the books Geralt was never described as incredibly ugly, just kind of unnatural and like a mutant with his pupils and pale skin color.
I think everyone in the first game was ugly as hell because CDRED didn't know how to model faces at the time.
I don't think Geralt would score nearly as much with women if he was that ugly.
