It's obvious that the trailer piqued a lot of people's interests, even those who aren't part of Kojima's following.
I got chills and I still do when the camera shot changes from Reedus' eyes to the five figures in the sky, good song too.
We know nothing about it other than what's in the trailer.

It just boggles my mind to see so many people making comments like "...I didn't get it!" or "That's it? No gameplay? Boring shit!"
Personally, I'm seeing tons of hints on the video alone:
All those "umbilical cords" attached to all dead animals and Reedus himself, those handprints appearing on the sand and on Reedus on their own, the post-autopsy looking scars on him, the USB-stick looking "dog tag" chain with mathematical stuff written on them, the scifi handcuffs, the five floating characters... weird and fascinating stuff!
It's safe to say that we can expect a psychological, and potentially philosophical experience. Something a bit akin to Talos Principle, cross-bred with Silent Hill and scifi, perhaps.

Boondock Saints came out in 1999.
Kojima is extremely good at imagery and surrealism.
If he focused more on that instead of his writing and exposition, Death Stranding could be his best game yet. I'm serious, say what you want about Kojima as a writer or director; he clearly has the mind of an artist.
