The top dog-tag is the equation for the Schwarzschild radius, which is used to compute the event horizon radius for black holes.
The dog-tag behind it is likely the covariant Dirac equation, which appears in quantum theory.
This is what happened:
As you all know, one of the most clear-cut (yet far-off) goals for physics is to unify the two main "realms" of physics - quantum mechanics (hmmm, the Dirac equation?) deals with the very small, and general relativity (hmmm, Schwarzchild radius?) deals with the very large/massive. The two theories are incompatible; they break down when trying to deal with gravity in the tiny, quantum sense (e.g. black holes, or the Big Bang).
But here's what happened. Man was able to create the largest particle accelerator ever conceived. It was so large, that it was required to be built in the ocean. Somehow, through tests in this particle accelerator, we were able to pick apart something that points to a "theory of everything."
But something went wrong. Horribly wrong. This explains all the dead sea life. The man is the only surviving experimental physicist, and he is stranded; likely, he is the only one with the answer. The baby, etc. were merely hallucinations; this man has lots of shock and trauma over what he dealt with (the baby is a symbol for his discovery; his "brainchild").
Something evil from a whole other dimension is stranded and dying in ours, fucking up everything and causing lovecraft-tier apocalypse. Reedus is a part of some team designed to send it back, he ends up going back to whatever negative dimension it comes from, which is why physics equations are engraved in metal around his neck. It's not known if physics is the same in that dimension.

I like that stranding theme, Kojima's concurrent with titles like that, he'll always use titles in some way like that. Not to box him in, but even Revengeance pretty much tells you what you're in for.
Death Stranding is about an otherwordly creature being "Death Stranded" here on Earth. Norman Reedus is a scientist, likely working for the government to examine and understand the creature's corpse. The loss of child in the trailer probably means Reedus' character lost his son recently which will feed into his psychosis examining the creature. I say creature but it's possible there will be multiple creatures, or that the floating people are the 'stranded".
If I had to guess I'd say Reedus is hoping the stranded come from some sort of afterlife where his son has moved on. In the trailer he 'follows' his son to the caucuses, and the floating. His hands also turn black implying he really enveloped himself in his work after his son's passing. The scar on his stomach could further hint at an alien influence and a past abduction. Reedus may assume his son was 'taken' by aliens and spent his life since trying to find them to find his son.

I didn't see anyone mention this, but most of the handprints are timed with the music and seem to be heading towards the baby until Norman picks it up. After he's picked it up, there are 9 beats (maybe 9 handprints) before the baby disappears. The baby's handprints then follow the same beat as the big hands, until they hit the sand, where they start following a faster beat. Also, The big handprints seem to have disappeared from the ground by this point.The baby handprints then head towards the sea.
What does this mean? I dunno, but I think the big handprints "took" the baby and have great importance.

Probably an observation made already but I put this in another Kojima thread.
The cords represent an entity's ties to their old world. The aquatic life have cords attached between themselves and the ocean, their old habitat before becoming beached. If the "Stranding" is indicative of something becoming stuck in a new place after arriving from another world and we take into account the Schwarzchild equation shown on his necklace, then the character has perhaps entered a black hole and ended up in this setting (or a different setting in the actual game, if this trailer is intended to be purely a symbolic piece). The baby and his scar are his ties to where he originally came from, though how they actually relate, I don't know. No theories as of now on the five figures in the sky.

Trailer gave away a few things of note. A couple things:
1) he's surrounded by a bunch of dead fish by a dead ocean. Maybe a catastrophe sent him here and killed all that sea life? Could be an environmental thing lik3 some other anon said, but maybe it's even a mass extinction.
2) he's covered in hand prints. Significant, but I'm not sure how yet.
3) the baby disappears from his hands and turns to tar. Then, handprints run down his leg. Could be related to the black sand, other hand prints on his body. The baby also has an umbilical chord. Think that finding his son/accepting his loss will be a main theme.
4) there are 5 figures on the horizon, above a dead, black sea looking towards this main character guy. These could be either allies or enemies, no way to know.
This area could he purgatory, considering the neutral, hopeless tones and all the dead stuff around. Maybe you play as a guy who goes through his life story? Maybe that's too obvious for Kojima?

Anyway this will be a post-apocalyptic scenario like EoE where he's either the last human or one of the last, and there's likely a rebuild theme going on. His dog tags have formulae on them for physics shit involving black holes and et cetera. The 5 figures are likely some kind of SEELE or even demiurge kind of group that polices the world.
We honestly can't know for sure until we play but I think it's safe to say that this has nothing to do with MGS6 and that Kojima's baby ruses are over. He's gone full David Lynch.
