The point of sparing him isn't to let him suffer as a worse fate, it's using him as a metaphorical catalyst for Niko to let go of his hate through.
Niko kills a lot throughout the series, but at that point, canonically, if you spare him, Niko is supposed to stop killing. Niko takes all the hate and anger that he has let boil inside him for so many years and just
lets it go.
And then he decides he'll begin again.
It's not a matter of whether killing him is bad or not, it's that Niko being able to stay his hand at that point is the first step towards him achieving peace with himself.
It all goes out the window at the end though.
