Status effects in general are pretty stupid. Most of the damage over time effects are stupid useless in hard / expert, doing all of 6 damage per second. Then you hit the pillar gear and suddenly get hundreds of damage per second, but it's still kind of shit when at that point you are doing thousands of damage per second with your attacks anyway. Shit like Venom and Cursed Flames should do a lot more damage than they do. Cursed Flames should work like a reverse shadow flame, having a long duration that deals more and more damage over time, venom could just do more damage in general I guess, maybe include a slow component or something.

Personally, I just want an accessory that provides light that wont get stuck in fucking walls and doesn't look ugly as hell. Or the ability to craft some buff potions into permanent accessories. More combined accessories, more accessories with unique effects like the shiny stone or spore sac....

The terrarian with a yoyo glove/bag? Yeah, that's fine. Lava Waders worked into my frost spark boots? Being able to see without having to slap down torches everywhere or chug a shine potion every 5 min? Nah, that'd be too OP.

The world should update every so often with new elements and things that happen that don't revolve around the player summoning a boss, like an army randomly moving in and creating their own area. But the game is mainly designed around ranged, with bosses depending on ranged abilities constantly increasing as the game goes by. The model of combat is essentially rushing horde enemies that do contact damage and projectile spamming bosses, they'd have to retool a lot to get melee away from becoming just another ranged type.

This. Starting a new character on expert was a blast, because the bosses felt so fair. The eyeball shield was an absolute necessity for dodging on the worm and the skeleton (I forget the names). it really felt you could dodge everything if you got gud. Then the skill ceiling gets vastly lowered with the hardmode bosses, and it becomes a game of circle around them while hitting them, and stacking as much regen as possible because their shots are basically impossible to completely avoid. You can't really git gud, you can only get better gear.

More enemy ai packages. There's only so many times they can get away with:
1) Enemy who moves towards player location
2)Enemy who moves towards the player location but isn't bound by gravity

Just scale it up even more. Turn corruption/hallow into a resourse - the larger the corrupted surface the more you can do/get. Make NPCs corruptable or whatever the hallowed equivalent would be and make sure there are pros and cons to this. Add spells and items so powerful they affect the whole world. Make invasions matter - goblins/pirates should steal your shit, vampires/werewolves should infect your NPCs, again with pros and cons. Make it possible to counter-invade those assholes. Add some sort of corrupt/hallowed invasion depending on which biome covers more land. Add bosses that destroy your world including stuff you built.

>Make invasions matter
At least in expert mode, NPC death should actually be something to avoid. Like, you have to ressurect them by burning x ores at an altar, or something. Right now it usually doesn't matter at all, even if your full base gets wiped, since they quickly and freely respawn anyhow.
