Disasters like these happened because of a time when E3 presentations were transitioning from an industry event to a mass media event and nobody really knew or was trained in how to deal with the changes in format.
It's pretty much impossible for things like Kaz's RIDGE RACER, Ubisoft's Jamie Kennedy, Cammy Dunaway or the whole Konami press conference to happen again. You might get nervous and clumsy devs not used to talk in public here and there, but most presenters are now highly trained to give proper speeches.
