I think there's something to be said for a system between. Let the community figure out what they think is acceptable with a ban system, or even letting an admin on their server decide what is acceptable, and zero behavioral moderation. I think some games may partially do this, but using reports as flags for "this player doesn't want to play with this type of player" rather than "this player needs to be punished because the community says so."
I don't think there's a perfect solution. Especially when a game's community can effect a game in the sense of marketing and therefore the developer or publisher can decide moderating their players is good for business.
