In video games this shit comes in as basically playing the game for you. You don't need environmental awareness because the game points enemies out for you. You don't have to be skilled at aiming, because the game aims for you. You don't need to be skilled manipulating the character because the game now does it for you. You no longer have to press the right buttons at the right time to fight characters because you can just hold down attack.
Worrying about whether or not a disabled person can play your game and building gameplay around that means they are going to make a much more simplified game. At what point do you say "ok, well, this person is way too fucking disabled to play our game. He has no legs, no arms, and he's deaf and near blind. be honest I liked the shooting and I felt that for the first time it was genuinely great, but there just wasn't enough of it. Uncharted 2 was a bit of a guilty pleasure for me and even though pretty much all the combat mechanics are better in Uncharted 4 it just doesn't play as well. Every time you kill a bunch of enemies you have another 10 minute traversal section so theres no sense of pushing through against the odds or that shoreline even give a fuck about killing you.
Besides that the main reason I got the game was to play co-op later on, I really loved the coop in U2 and U3, the horde mode in U3 was one of the best I've seen in a game too.

For one, video games aren't a hobby in the same way watching TV isn't a fucking hobby. It's entertainment.
Two, app;y that to other games people play in the real life. Basketball, baseball, football, For the disabled to get the same experience means to change the rules for everyone else as well. No dribbling in basketball, flopping about in football, and unless extremely dedicated to the sport, baseball is out. You can give them special permissions but they aren't playing the same game anymore.
So what happens is the "creators" change it so everyone has the same "accessibility". Making things accessible usually requiring less precision or movement. Which means more QTE bullshit and taking control from the player. The devs COULD design it both ways but that literally means more testing, more scripting, more animations, and many more things that are not cost effective. So rather than do both they just go with the option that works "both ways".
Again, there are somethings more complicated than adding a menu option to change it's "accessibility".
