Rarely use companions actually, normally do their quest and then send them on their way to a hopefully better life.
Aside from bullet sponge robo-scorps I've never really had difficulty with anything.

Yeah, I fucking love it, will never play without it. It's really modular too so you can customize it to be exactly how you like..
Basically it makes it so the shit you have on you can degrade (ie food, water, ammo, spare parts) can all break down due to getting shot, getting wet, being on fire, radiation, or just weathering due to time. This makes it so you have to restock every so often because buying in bulk is kind of wasteful.
It also slashes prices across the board for stuff, making the caps you get as quest rewards actually very valuable. I found myself actually looking for work in freeside so I could get into the strip or just eat, when I never had that problem before. I found myself having to steal or hoard expensive items so I could sell them to get some ammo or weapon mod.
I really enjoy it, highly recommend
