The combat is pretty good, it's all action based. However you need a pretty good ping to be able to play well, and especially if you play a style that uses animation cancelling, it will directly affect your damage output.
The long-term game is shit, however, with a lot of grind necessary. Also servers are overrun with bots that constantly mess up the economy (at least up to one month ago, when I stopped playing, and I doubt they have been taken care of), the game has major performance problems, if you're in Europe there are at time known routing issues that will make your ping higher (in my case I normally had 24ms and when the routing problems presented themselves I'd bounce between 35 and 50).
Regardless, the story mode/main quest is worth trying out (although still incomplete at the moment) and while you're at it you'll get an idea if you want to keep playing.
Finally, real money only speeds up the grind, there is no P2W.

It is, but the way models are generate is as many pieces put together. So when an NPC gets generated it's assigned hairstyle #3 with outfit #5 and face #1, for instance. Now, what I assumed is that most of that information about NPCs isn't saved, so when the switch between low and high detail happens they get regenerated and can get different looks.
