You do the same amount of damage as you do in other modes in my experience at least.
The Enemies do dish out more damage though, but there are also more of them, and they act more aggressively. Imps will actively try to flank you constantly, toss their bullshit fireballs at you, even from behind cover so that you can't shoot them, and in general, try to be as big pain in your ass as possible on Nigthmare.
Seriously, a lone imp can fuck you up if he flanks you.
