So what does that make the current firelink? When I first got to the abyss firelink I freaked out a bit since something the handmaid said made me think it was all an elaborate hoax or something. Firelink is the only place you can't physically travel to. She mentions already being held captive or something to that effect so it just vaguely gives me the idea that the abyss ending might have already occurred and we were essentially playing an alternate timeline or an alternate dimension that just shows how unkillable the first flame is. Then the firkeeper mentions something about our grand betrayal and I'm even more confused.

So I just want to say Company Captain Yorshka makes no fucking sense is a walking Retcon. She says she's a daughter of Gwyn, but not even vaguely referenced in Dark Souls 1. Somehow, she took over the Blades of the Darkmoon from Gwyndolin, doesn't even seem to care that Gwyndolin's dead. Come to think of it, why didn't she fight off Aldrich and Sulyvahn? And worst of all, she's partly dragon. Which makes the idea that she's related to Gwyn fucking laughable because Priscilla had to hide in the Painted World so Ornstein didn't kill her ass.

That's because it is, the original firelink was swallowed by the abyss, in a last effort Ludleth linked the fire and changed the timeline so we're now sitting in a completely fine firelink. It's implied that the abyss ending happened during Ludleth cycle, but as Aldia said in DaS2 the flame never fades out entirely, even when the abyss consumes everything there will still be a a little left of the flame so that one day someone can link the fire again.

Nameless King is pretty much confirmed to be Gwyn's firstborn based of his soul and weapon descriptions. He has dragon slayer armor because he was once a dragon slayer and was Ornstein's mentor. Ornstein's armor description in DaS3 implies Ornstein left Anor Londor in search of the firstborn/nameless king.
My own theory is that you find Ornstein's armor there because he went in search for Nameless and fought with Nameless to try to bring him back and got his shit fucked up

Unkindled are piles of ash that are resurrected by the Bell of Awakening with the purpose of finding Lords of Cinder to fuel a linking of the flame. Each one was a real person at some point. It's not different from an Undead because Unkindled and Undead aren't mutually exclusive states. In general though, you can't become Unkindled, but you can become undead. Most Unkindled are also Undead.

Not sure if I'm right here, but here's my few thoughts. In the first Dark Souls when you kindle a bonfire you're sacrificing humanity to increase the fire's strength. If the player is "unkindled" then they might not have any humanity to them (since the dark soul of manus where humanity originated as since faded/gone out). Basically an unkindled may be an undead without humanity.

There's one area in the Smoldering Lake that's almost IDENTICAL to the section in the Depths where the rat leads you into a pit of basilisks, but instead it's lava this time around. Also, Smoldering Lake itself is full of rats, basilisks, and those Demon Ruin statues.

Each chest is probably a unique entity with a flag that shows when it's item is taken. They probably took the firelink shrine map, copy-pasted, and just changed the lighting/enemies and deleted most of the NPCs.

I don't think that's the intent. It's more that the world you warp to is a fucked up, dead world that is beyond saving, and you warp in from your world through Firelink Shrine to plunder it for the pieces necessary to save your own world.
I always thought that the way the handmaiden talked in the Untended Graves was meant to be an implication of that. The Untended Graves going quiet from no souls remaining, and that other line about you and the fire keeper being tied to that world or whatever. I took it as her saying "This world is long beyond saving, but I see you've come here to save your own" or something like that.
TL;DR We're scavenging a world that is just completely fucked, in an attempt to keep ours from ending up the same way.

The rats, basilisks, and poison goat people seem to just inhabit the shittiest areas of the planet. The Smouldering Lake being a gigantic underground battlefield filled with decaying corpses, it's no wonder they're all over the place.Smouldering Lake was a Izalith style lava realm until it was flooded by Yhorm in his quest to destroy the Chaos demons. In doing so he acquired the Profaned Flame and set the Profaned Capital on its path to destruction. The goat pyromancers are simply rediscovering the pyromancies of the old demons, with the Old Demon King being a particularly powerful goat pyromancer who mantled the spirit of a shitload of power chaos demon souls, as can be seen by his gigantic sacrificial piles of demon corpses.
